Student Registration

Residency
Families, employees, students, and concerned citizens are encouraged to report suspected abuse of the Seekonk Public Schools Residency Policy by emailing the district's Residency Officer at residency@seekonkschools.org.

“Residence” is the primary place where a person dwells permanently, not temporarily, and is the place that is the center of his or her domestic, social, and civic life. Temporary residence in the Town of Seekonk, solely for the purpose of attending a Seekonk public school, shall not be considered residency.

In determining residency, Seekonk Public Schools reserves its right to request a variety of documentation and to conduct an investigation into where a student actually resides. Because residency can, and does, change for students and their families during the academic year, Seekonk Public Schools may continue to verify residency after the commencement of classes. Also, Seekonk Public Schools may act upon anonymous tips it receives to conduct a residency verification investigation.
  

How to Register New Students

New pupils planning to attend the Seekonk Schools, but who have not registered, should contact the school directly.  All registration and residency paperwork must be turned in directly to the school at which your child will attend. Registration during the school year can be done at the school during the following hours:

Grades 9-12:  Seekonk High School Guidance Office, Hours 7:30 AM to 3 PM (508-336-7272)

Grades 6-8:  Hurley Middle School Main Office, Hours 8 AM to 3 PM (508-761-7570)

Grades PK-5:
Aitken Elementary School Main Office, Hours 8:30 am to 3 PM (508-336-5230)
Martin Elementary School Principal's Office, Hours 8:30 am to 3 PM (508-336-7558)

Please call the appropriate office for dates and times for summer registration.

To Register Your Child

All of the information below is available in the school offices listed above.  If you would like to complete the forms in advance, copies of some of the forms are available below.  These are the forms and information that are required to register your student:

This form is needed to request records
from a previous school or to provide
information to another school should
your family move.
  4. Residency Forms
All families need to complete these forms,
some of which will need to be notarized. 
If you have multiple children registering,
all their names can be put on one form.  
  5. School Nurse Forms
Forms that provide important information
for the school nurse.
  6. Certificate of Immunization
Required by the state.
  7. Massachusetts School Health Record
Required by the state
  8. Over the Counter Medication Protocol
This form provides the nurse with
information on what medication your child is allowed
or not allowed to have while at school.
  9. Transportation Registration
Required if your child will be riding the
bus.
10.  Previous School Records
Students in grades 2-12 must provide
previous school records including report
cards.
11.  Birth Certificate
Please bring in an original birth certificate.  The school office will make a copy.
